Adult Executive Function Assessment

 

Instructions: This assessment is designed to evaluate 12 executive functions. Answer each question based on how often the statement applies to you. Use the following scale:

1 – Never

2 – Rarely

3 – Sometimes

4 – Often

5 – Always

Response Inhibition (Self-Control)

1. I find it difficult to stop myself from saying things impulsively.
2. I interrupt others during conversations without thinking.
3. I make impulsive decisions that I later regret.
4. I struggle to resist distractions when working on tasks.
5. I struggle to manage money and/or delay gratification.


Working Memory

6. I have trouble remembering instructions with multiple steps.
7. I forget appointments or deadlines unless I write them down.

Scoring & Interpretation: Lower scores indicate stronger executive function skills, while higher scores suggest areas for improvement. This assessment can be used to identify challenges and develop strategies for growth.
