Comprehensive Young Adult Therapy in Bethesda, MD

Ascend’s young adult intensive outpatient program has been restructured to offer greater flexibility and improved affordability. For more than 14 years, Ascend has been a trusted leader in young adult IOP care in the DC metro area, helping hundreds of young adults and families navigate anxiety, depression, executive-function challenges, and the transition to independence with lasting results.

Ascend integrates evidence-based therapy, executive function coaching, social and life-skills practice, and academic and career readiness into one cohesive, in-person program. Care is delivered by experienced clinicians and educators who specialize in young adult development, with a small staff-to-client ratio that allows care to be tailored to each young adult’s specific needs, pace, and goals.

We are intentionally not virtual. While online programs can inadvertently reinforce avoidance and isolation, Ascend helps young adults build confidence, accountability, and resilience through daily structure, authentic connection, and guided practice in the real world.

Ascend is intentionally independent. Our self-pay model allows care to be guided by your young adult’s needs rather than insurance requirements or fixed timelines. This flexibility lets treatment move at a pace that reflects real progress, adapts as challenges and strengths emerge, and supports meaningful, lasting growth.

Who It’s For

Young adults ages 18–28 who need structured, comprehensive support to manage mental health challenges, strengthen executive functioning, and move forward into adulthood with greater stability and confidence.

How It Works

Ascend is a three-stage treatment program requiring a minimum of a three-to-six-month commitment.

Stage I: The Buy-In

  • Identifying treatment goals
  • Making connections
  • Committing to the call for change

Stage II: Becoming the Change-Maker

  • Implementing tasks to reach goals
  • Developing self-awareness
  • Challenging old, useless, self-narratives
  • Facing fears head-on

Stage III: Integration and Ascent

  • Creating a life worth living in accordance with core values
  • Deepening relationships
  • Expanding social support system
  • Practicing new skills in daily life that build lasting stability and long term success.

Progress is reviewed monthly in treatment team meetings.

Ascend helps young adults integrate new skills into everyday life, build deeper relationships and support, and create a life worth living aligned with their core values.

Integrating Family Health Into Every Stage of Treatment.

Family involvement is central to Ascend’s work. Parents are engaged from the start, participating in intake, sharing essential history, and attending twice-monthly Parent DBT and Support Groups focused on communication, boundaries, and steadiness at home. Families also join initial and final treatment planning meetings so goals, progress, and next steps are clear and shared.

At Ascend, we understand young adult growth as part of a larger family system. We help parents make sense of the developmental process, step out of reactive patterns, and support independence with more clarity and confidence. The result is not just change during the program, but a family better equipped to sustain it long after treatment ends.

Pathways: ASCEND Young Adult Intensive Outpatient Services

Ascend Lite : Young Adult Therapy & Skills-Based Therapy for Young Adults

Ascend Lite is a structured, lower-intensity option for young adults ages 18–28 who either need continued support after a higher level of care or benefit from a more flexible alternative to full Ascend IOP. Placement is based on each young adult’s clinical needs, level of independence, and readiness for structure.

Participants attend 3–6 hours per week of in-person, individual therapy and skills-based therapy groups focused on emotional regulation, executive functioning, and interpersonal effectiveness, while remaining engaged in school, work, or independent living.

Ascend Lite groups are an effective way to deepen progress alongside an existing individual therapist or treatment team, adding structure, skills practice, and peer accountability that strengthen ongoing work.

Groups May Include:

  • Process Group

  • Goals Group

  • Dialectical Behavior Therapy (DBT) Group

  • Cognitive Behavioral Therapy (CBT)/Exposure Group

  • Art Therapy Group

Ascend Lite Is Best For Young Adults Who:

  • Need more support than weekly therapy

  • Step down from IOP, PHP, or residential treatment

  • Would benefit from the full Ascend program but are unable to participate due to space or scheduling constraints

  • Are looking to expand services and supports and continue work with existing providers

Participants may continue with existing individual therapists, psychiatrists, or other providers. If needed, MCA can provide individual therapy and will coordinate care with outside providers.

Family Support

We strongly encourage parents to join our twice-monthly virtual DBT/Support Group, designed to help parents better understand and support their young adult children through mental health challenges.
